Getting started

Setup takes about five minutes. The Home page in the app shows the same four steps and ticks them off as you go.

1. Install

Install Form Expert from the Shopify App Store. The app opens on the Free plan; no card is needed.

2. Choose who gets notified

1. Open Settings in the app.
2. Under Notification recipients, add the email addresses that should receive submissions. The Free plan allows 3 recipients, Starter 5, Pro 10, Scale 15.
3. Save. Replies to notification emails go straight to the person who submitted the form.

3. Create and publish a form

1. Open Forms.
2. Click Use template on a template (Contact us is a good first form), or New blank form, or describe the form in the AI box and click Draft with AI.
3. The editor opens. Adjust fields if you like, then click Publish.

A form only appears on your storefront once it is published. Saving without publishing keeps a private draft.

4. Add the form to your theme

Click Add to theme in the editor. In the theme editor, add the Form Expert form block to any page section, pick the form in the block settings, and save.

5. Send a test submission

Submit the form on your storefront. Within a few seconds:

• Submissions lists it with a New badge.
• Delivery Log shows the notification to each recipient with status Delivered (or Sent while the email provider confirms).
• The email arrives in the recipients' inboxes.
